Social Prescribing Community Connector - Richmond

Program helps older adults explore community services and activities that can improve their health, well-being, and independence.
The Social Prescribing Program at RCRG helps seniors access non-clinical, community-based services to support aging in place through a comprehensive, strength-based approach that fosters social support.

How It Works
  1. A health care professional, family member, or self-referral identifies an older adult who could benefit from non-medical community-based programs or services.
  2. The senior is referred to Social Prescribing at RCRG using the referral form (see below).
  3. A social prescribing community connector receives the referral and assists the senior in accessing local programs and services to support their health and wellness. This may include social or recreational programs, support for aging in place, mental health services, nutrition services, etc.
Eligibility
  • Seniors 60 years of age or older
  • Reside in Richmond
  • Experiencing or at risk of Social isolation and/or loneliness
  • Experiencing or at risk of Barriers to accessing community-based resources
  • Experiencing or at risk of Low income
  • Exceptions made for those aged 55 or older with chronic health conditions
Referral Instructions: Please send a completed referral (see below) form to the Senior Community Connector for intake and processing.
